Nurse

Job Description 

The Nurse is responsible for managing patient care plans according to physicians’ instructions, administering IVs, changing dressings, cleaning wounds and updating Doctors about their patient’s health in both at clinics and at home. 

Duties and responsibilities 

• managing their care plans according to physicians’ instructions.

• Assess and chart observations of the patient’s condition at each visit.

• Complete evaluation tasks, including reviewing medication and vital signs.

• Monitoring, addressing, and documenting the symptoms and changes in patients conditions 
• Using appropriate assessment tools regarding clients well-being and care

• Administer Physician-prescribed medication.

• Administering medication and insulin, and completing blood pressure, glucose, urine, and stool tests.
• Inspecting wounds, changing dressings, and handling personal grooming and hygiene.
• Provide catheter care
• Ensure patient is fed and bathed
• Ensure bedding is clean
• Listening to the concerns of family members and answering their questions.
• Testing for muscle weakness, bedsores, and any signs of infection.

• Provide education to patients and families on proper home health care procedures and strategies.

• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.

• Keep the patient comfortable through palliative care.

• Recommend tools or devices that may improve the quality of life for the patient.

• Performs other related task given by supervisor.

Job requirements

Educational level

• University degree in nursing from an accredited academic institution 
• Relevant license and certification.

Working experience 

• Minimum 6 months  years of experience
• Additionally, any experience in home care or nursing homes can make an applicant more competitive.

Professional knowledge and skills.

• Excellent report writing skills.

• Strong communication skills

• able to deliver skilled services like assessment and observation, wound care and dressing changes, injection administration. IV monitoring, tube feeding, catheter insertion and monitoring, tracheostomy care, medication administration, patient and caregiver education.

• Active listening skills and empathy.

• Physical and mental strength.

• Critical thinking which means ability to integrate information, outcomes, experiences, and other evidences and use that data to identify and solve problems either independently or in conjunction with the patient’s care team.

Work Environment

• Must be willing to work on shift basis; nights, weekends and holidays as well as normal daytime hours.
• Must be able to calmly handle angry and unhappy customers.
• Able to sit in front of and work on a computer screen for long periods of time.
• Must be able to work in high pressure, stressful situations.
• Ability to handle stressful situation appropriately.
